In Salvation, Tom arrives late to the meeting wtih Meg and John Winchester for John to hand over the Colt, because he was slicing John's truck tires. Meg demands the Colt, and John hands it over. Unsure if it is the real Colt, Meg hands it to Tom. He looks it over, and cocks it - all the while eying John. Turning, Tom shoots Meg. She doesn't die, and he declares the gun a fake, throwing it away. Meg and John share some witty banter, before steam escaping distracts the demons enough for John to run. Tom and Meg follow, until they catch up to John in the access tunnels where John turns holy water on to spray at them. Tom starts to walk across the grating floor, but the holy water keeps him back. John smiles, and seems to escape. Later on, however, Tom finds John in an alley and throws him against the wall, holding him tightly there while raising John up about 3 feet. The demons have captured the great John Winchester.
Tom tackles Sam in Devil's Trap, as the Winchesters are trying to get away, laying Sam flat. He hits Sam twice before Dean kicks him in the face to try to dislodge Tom off of Sam. Tom uses his demon powers to toss Dean into a car's windshield, before pummeling Sam's face eight times. Dean uses the Colt to shoot Tom. Briefly, Tom's skeleton is seen, then he falls off sideways, dead.
Azazel in in In My Time of Dying makes a comment that Dean killed some people very special to him - implying Tom, who was killed with the Colt, and Meg, who was exorcised. He implies that he wants Dean dead almost as much as the Colt, because of this.
