Dean and Sam's first encounter with a reaper was when Dean was fatally injured in Faith. Sue Ann LeGrange had bound the reaper sent after her husband, Rev. Roy LeGrange, and was using it to heal who she thought worthy and killing the unworthy.
After a car accident in which Dean hovers between life and death, he encounters a grey spirit in In My Time of Dying. After realizing this spirit - a reaper - is taking people when their time is up, Dean meets Tessa. When he discovers she is this reaper, she tells him his time is up and she is there to take him. He at first refuses to go, but when he makes up his mind Azazel possesses her and sends Dean back into his body - whole.
Reapers look human and there as many different looks as humans have. The reaper in Faith looked gray and cadaverous while Tessa looked like a light gray ghostly form. Tessa was also able to take a beautiful woman form in her other appearances, and Billie was also able to take on a beautiful woman in her appearances.

"The physical manifestation of death, the personification of this most feared event, is a concept that has existed since the beginning of recorded history. The most common representation of death - aka the Grim Reaper - is a skeletal figure carrying a large scythe, and wearing a flowing hooded black robe. The origin of this imagery appears to be related to Father Time, since there is a direct correlation between death and aging, and the Greek personification of time, Chronos, is represented as carrying a sickle (due to storytelling confusion with Cronus, a harvest god.)"
"While the thought of death coming for you can be truly terrifying, reapers aren't usually depicted as murderous monsters. They just collect souls. Often they can be quite helpful, aiding the souls on their journey to the next world. Though, while the reapers don't distinguish between good and evil when choosing whose life to take, they still note whether your soul glows black or white, so they know where to take you..."
