Sue Ann LeGrange helps her blind husband up to the lectern. She stands to the side, listening to his sermon and the banter between him and Dean Winchester. Roy calls Dean to the stage to be healed, and when he finally comes forward, she steps over to help Dean up to stand next to Roy before stepping away.
The next day Dean shows up at the LeGrange's house, asking questions. Sue Ann pours some iced tea before she sits. Roy and Sue Ann answer Dean's questions about the miracles, what had happened to Roy, God blessing Roy by healing people, and how much the flock is growing. Dean asks one final question - Why me? Roy tells him that he looked into Dean's heart, and that he is a young man with an important purpose with a job to do, and it isn't finished.
After Dean's visit, Sue Ann helps Roy lay down to rest before going out on the porch to meet Layla and her mother. She informs them that Roy is resting, and won't be seeing anyone else. Mrs. Rourke is frustrated, and Sue Ann tells her that Roy is aware of Layla's situation, and will help her just as soon as the Lord allows. After telling her to have faith, she turns and heads back inside.
At the next service, it is assumed Sue Ann helps Roy on stage, and then later the elderly man with emphysema up on stage with his wheelchair, before she steps off to the side.
Before the next service, Sue Ann, with the help of another man, helps Roy leave the house and head over to the tent church. She stands to the side of Roy, until he calls up Layla. Dean stops Layla to talk with her, but she eventually tells him she's sorry and heads for Sue Ann, who helps her on stage and stand next to Roy before stepping away behind the piano. Before Roy can heal Layla, however, Dean calls out "Fire" and the tent is evacuated. Sue Ann continues to do her praying in Latin for Layla to be healed; however, Dean finds her and stops her. She panics, calling for help. The deputy shows up, and hauls Dean away. He is joined by another deputy, as they haul Dean outside of the tent. Sue Ann walks around him to talk about how disappointed they are in him, and how the Lord will deal with him. She tells the deputies she isn't going to press charges and lets him go. She walks over to stand next to Roy, who is telling Mrs. Rourke that he will heal Layla tonight.
Before the healing of Layla, Sue Ann does her black magic so Dean will be the next one killed by the reaper. While Roy is working on healing Layla, Sue Ann faces Sam in the basement. She tells him she gave Dean life and she can take it away. Sam overthrows her altar, while she makes good her escape and traps him in the basement. She tells him that the Lord chose her to reward the just and punish the wicked, and that Dean is wicked and deserves to die as Layla deserves to live. She tells him goodbye, slipping away to pray to the reaper just outside of the tent.
Before Layla is completely healed, Sam shows up and breaks the blood with her cross, thereby breaking the hold Sue Ann has over the reaper. She is horrified at what he has done, before looking up and seeing the reaper. She tries to run, but he is there, taking her life. She falls over, dead.
